Some referrals to the Macular Service can prove inappropriate and delay the urgent assessment and treatment of patients who do have wet ARMD. The recognition of the signs and symptoms of neovascular ARMD is vital. The discrimination of cases of true neovascular ARMD and the cases of ARMD mimics is the key. The evalutation of treatment outcomes can help in future delivery of of the service. These outcomes are monitored.

Since the advent of the Snellen chart in the nineteenth century, almost every routine eye examination has included some form of visual acuity measurement using a letter-based chart, and the use of letters as targets during refraction. Improvements in acuity chart design, such as logMAR testing, have greatly improved standardization, but recent studies have indicated that the repeatability of conventional letter charts remains poor at around 0.2 log units. This limits the ability to reliably detect change in visual acuity, either following refraction, surgery or medical therapy. One reason for the poor repeatability may be differences in the spatial frequency content of letters, particularly at the low frequency end of their spectrum. Recently we have been experimenting with new letter charts, employing high-pass filtered (Vanishing Optotype) letters and other targets, in an attempt to improve the reliability of acuity measurements and refraction procedures. This lecture will review the history of acuity testing, the limitations of current techniques and some recent developments in chart design.

Our treatment of ARMD has advanced dramatically over the last decade resulting in the opening of the new Macular Service in 2009. This lecture will outline our current indications for treatment in ARMD in particular discussing the various presentations and clinical features of patients with neovascular ARMD with clinical examples. The presentation will also discuss conditions which mimic ARMD. These are conditions that are frequently referred and have features similar to ARMD but are not yet being treated by the current macular service.

This lecture aims to outline the current conditions requiring vitreoretinal surgery, their clinical features, outcomes and when intervention is required. Discussion will be focussed around retinal detachment surgery, diabetic vitrectomy and vitrectomy for macular diseases.
